Title: Help with custom editor "input file"

Im trying to use a custom editor, But I can not get it to work properly. when i click the button "choose a file",the editor disappears, and when i click in accept button, nothing happens.

im recreate it in a jsfiddle

Is this the right way to create a custom editor?

Any idea how to solve this?
